Looking to Book an Appointment?

To schedule an appointment, please call 718-554-3680 at any time between 8 AM and 6 PM from Mondays to Thursdays, and from 9 AM to 12 PM on Fridays. You can also stop by our office, located at 2919 Avenue S to schedule an appointment in person. Please have your insurance card, ID (or some form of identification, if you are scheduling for your child), and your referral form/prescription from your doctor.
